2007-08 Intercollegiate Athletic Sports Insurance Program
Important Forms:
Please take time to read this letter in its entirety regarding the sports insurance procedures because there are issues that could affect your child’s ability to participate in Walsh University's Athletic Program.
- Your private insurance is considered first and primary for all athletic claims. No athlete will be allowed to participate in any intercollegiate sport without submitting a signed insurance waiver form to the Business office
- All athletic injuries must be reported to an Athletic Trainer as soon as possible so that a claim form can be filled out.The athlete and/or parent are responsible for making sure claims are filed with 30 days of the accident.
- After your private insurance has processed your claims, if there is a balance due, you must forward copies of your company’s E.O.B. (Explanation of Benefits) to: The Baker Agency, Inc. P.O. Box 390 Plainwell, Michigan 49080.For claims assistance please call 1-800-223-1318.Most claims are paid at 100%: however in some instances there may be a balance left over.
